New Member Application

In order to apply for membership in TOA, please complete the following form.  Please note, only former TOA members who have attained Regular member status may apply as Regular, Senior Associate, or Emeritus members.  All other ophthalmologists must apply as Provisional, Resident, Military, or Out of State as appropriate.

REGL (reinstatement application)
     Regular Member (Voting Member - $550 Annual Dues): Ophthalmologist licensed in the state of Texas who has been a TOA member for more than one full year and has been elevated from Provisional.
PROV (new application)
     Provisional Member (Voting Member - $300 Annual Dues): Ophthalmologist licensed in Texas.  A Provisional member may be considered for elevation by the Executive Council after a full year of membership.
OUTS (new or reinstatement application)
     Out-of-State Member (Non-voting Member - $75 Annual Dues): Ophthalmologist practicing in another state or foreign country.  Texas licensure is not required.  Those Out-of-State members who are in a foreign country will not receive physical mailings, but will receive all email communications.
SENA (reinstatement application)
     Senior Associate Member (Non-voting Member - $25 Annual Dues): A former Regular member who through reason of age or disability has become relatively inactive, reducing his or her practice by half or more.
MILT (new or reinstatement application)
     Active Duty Military Ophthalmologist (Non-voting - $0 Annual Dues): Ophthalmologist in Active Duty service in the United States Armed Forces.  Texas licensure is not required.
EMER (reinstatement application)
     Emeritus Member (Non-voting - $0 Annual Dues): Former Regular member who has completely withdrawn from the practice of medicine due to age, disability, or any other good and sufficient reason.  If an Emeritus member resumes practice, he or she must request a change in member status.
RESD (new application)
     Current Resident or Fellow (Non-voting - $0 Annual Dues): Physician enrolled in an approved ophthalmology residency training program in the state of Texas, or in a subspecialty fellowship at such a program.
